That's not a defensive wound, that's an attack. If he'd grabbed the knife and randomly stabbed her in self defense, his spin would be more believable. It's pretty tough to slit someone's throat in the middle of defensive panic after being attacked with a knife. It's a move that strikes me as coming from behind. I hear it's tough to slit a throat, that you really need the right leverage and pressure. Tough to get the right leverage and pressure from the front; it's easier to pull and cut than push and cut.
