There's some overlap, but the primary purpose of a plan is to limit the amount of coverage for a specific calendar year.
Many plans provide insurance up to a capped expenditure per year, others provide a capped set of procedures and set a plan rate for payments.

An example:

Plan A provides a cap of 2000 dollars of benefit per calendar year.

Plan B covers the first 1500 dollars of 6 fillings per year and 100% of two dental cleanings, along with one set of wing bite X-Rays and frontals.

Plan C covers 50% of the first 1000 dollars of coverage then 80% of any overage up to 3000 dollars.

These plans are picked over and chosen to cover the best benefit. I was born in the sticks, so I didn't have consistently fluoridated water, add that to my Cherokee heritage and I can floss my gums bloody and still walk out of a Dentist office with a few fillings.

I prefer insurance over the plans, in that they typically are structured as something like, 80% of one, 50 % on orthodontics, 100% on cleanings, flat deductible.
